Physical Therapist Job in Kendallville, IN

Join Parkview Health At Home in Kendallville as a Physical Therapist. You will deliver patient care across four counties—Noble, Lagrange, Steuben, and DeKalb—serving primarily rural areas and mid-sized towns.

Schedule & Compensation

• Monday through Friday, 8:00 AM to 5:00 PM (5x8 Days)
• $2,190 weekly
• 91-day assignment
• Start date: June 22, 2026

Your Role

You will complete 4–6 patient visits daily using a point system and a tablet to track time and visits. Your clinical responsibilities include performing OASIS Assessments and Start of Care cases. Common patient presentations include elderly falls and fractures. You will use the Homecare Homebase (HCHB) charting system for documentation.

Requirements

• Current Physical Therapist license (verification required)
• BLS certification (American Heart Association)

Dress Code

Business casual or Hunter green scrubs with white or black top.

Setting

Home health care delivery across a multi-county region. You will work in patient homes and community settings rather than a traditional clinic or facility.
