Tide Health Group – Georgian Gardens is seeking a Speech-Language Pathologist (SLP) for a long-term care setting in Potosi, MO. This position offers a unique blend of clinical practice and leadership responsibility, ideal for an experienced therapist ready to step into a Director of Rehabilitation (DOR) role.

Role Overview

You will serve as an assessing therapist with a lighter caseload while assuming DOR responsibilities. Your day-to-day will include attending morning meetings and facility meetings, managing rehabilitation team scheduling, and overseeing clinical and financial performance. You'll provide leadership and direction to a multidisciplinary team including PT, OT, speech therapy, PTA, and COTA staff, fostering collaboration to optimize patient outcomes.

Key Responsibilities

  • Conduct patient assessments and therapy services with a reduced caseload
  • Lead the rehabilitation team and ensure compliance with PDPM (Patient-Driven Payment Model) processes
  • Develop and enhance rehabilitation programs for high-acuity patients
  • Manage regulatory compliance with federal, state, and facility standards
  • Monitor clinical and financial performance metrics
  • Coordinate interdisciplinary care planning

Schedule and Benefits

This is a day shift position: 5 days per week, 8-hour shifts. No on-call responsibilities and no weekend shifts required. The assignment is 91 days, starting July 27, 2026. Weekly pay is $2,534.40.

Requirements

  • Current Missouri state SLP license
  • Minimum 1 year of SLP clinical experience
  • Prior leadership experience within a skilled nursing facility (SNF) or long-term care rehabilitation setting
  • Demonstrated knowledge of PDPM
  • New graduates are not eligible for this position
