About this role
You will deliver skilled physical therapy services in a home-based hospice setting, working with VNA Hospice of the Southwest Region. This is a full-time position working 5 days per week on a day shift schedule.
Your Key Responsibilities:
- Conduct comprehensive patient assessments covering functional status, safety, and risk factors in accordance with APTA guidelines and federal and state regulations
- Develop individualized care plans with patient-specific goals and therapeutic interventions that promote functional independence and safety
- Deliver skilled physical therapy treatments, including therapeutic techniques and patient and caregiver education, while monitoring patient progress on each visit
- Manage and optimize patient visit schedules to maximize productivity and meet evolving patient needs through rescheduling and load adjustments
- Supervise and direct clinical activities of physical therapy assistants, PT students, and home care aides in compliance with regulations and APTA guidelines
- Collaborate with physicians, nurses, caregivers, and rehabilitation services to coordinate comprehensive patient care
- Complete OASIS time points and all required clinical documentation that accurately reflects care provided and outcomes achieved
- Perform patient health assessments including PT/INR and blood pressure monitoring
- Execute patient-related procedures including staple and suture removal and wound care
- Ensure accurate billing and charging processes, with documentation supporting charges and insurance coverage compliance
- Coordinate safe patient discharges and transfers with appropriate paperwork processing per department policies
- Participate in peer review audits and provide constructive professional feedback to team members
- Collaborate with quality improvement staff to enhance accuracy of clinical notes and OASIS documentation
- Contribute to in-service training programs for rehabilitation, medical, nursing, and volunteer staff
