Physical Therapist Home Health Job in White River Jct, VT

You will work as a full-time Physical Therapist in home health, providing direct patient care in the White River Junction, Lebanon, Bradford, and Windsor areas of Vermont and New Hampshire. This position offers day shifts with meaningful engagement—each patient visit averages 45 minutes, allowing you to deliver comprehensive therapy in the comfort of patients' homes.

Your responsibilities include:

• Screen, evaluate, re-evaluate, and treat patients in their home environments
• Develop appropriate treatment plans and carry out treatment programs tailored to individual needs
• Document evaluations, treatment goals, and objectives according to regulatory standards
• Teach patients and family members the rationale for treatments and procedures
• Coordinate and communicate treatment activities with interdisciplinary team members
• Work with broad independence while remaining part of a cohesive patient-care team

Required qualifications:

• Bachelor's degree in Physical Therapy
• Current, unencumbered license as a Physical Therapist in Vermont and New Hampshire, or valid compact license
• Minimum one year of experience as a PT in a home healthcare setting
