PT - Physical Therapist

Job Details:

• Outpatient physical therapy setting
• Day shift hours from 10:30 AM to 6:00 PM
• 8-hour shifts
• Dress code: Khaki and White Polo
• Required certifications: BLS certification from AHA
• Must have current New Mexico Physical Therapist licensure
• Must be proficient with EMR and EPIC systems
• Bachelor's, Master's, or entry level Doctorate of Physical Therapy degree from an accredited college or university required
• Proficiency with all modalities and equipment used by a Physical Therapist required
• Perform comprehensive diagnostic evaluations to assess physical and cognitive abilities of patients
• Develop and implement individualized treatment plans with measurable goals in collaboration with patients and families
• Provide follow-up and discharge planning for referred patients
• Maintain thorough documentation including initial patient evaluations, daily and weekly progress notes, and discharge summaries
• Participate in patient care conferences, family conferences, in-service programs, and departmental meetings
• Counsel patients, families, and involved individuals regarding patient evaluation, care, and treatment
• Assist with coordination and supervision of Physical Therapy and Physical Therapy Assistant student programs
• Supervise Licensed Physical Therapy Assistants, Rehab Technicians, and Volunteers
• Assist with clinical aspects of the department including orientation of new associates and scheduling of patients
• Participate in Performance Improvement activities and maintain departmental productivity standards
• Maintain awareness of community agencies to make appropriate referrals when indicated
