Job Details:

• Minimum 1 year of experience required; travel experience required
• Valid California Physical Therapy License required
• BLS certification required (AHA or ARC)
• Valid driver's license and auto insurance required
• EPIC experience required
• Serve adult and geriatric patient populations
• Conduct up to 5 patient visits per day
• Collaborate with Physical Therapy and Social Services interdisciplinary support teams
• Utilize critical thinking and complex problem-solving skills with the ability to work independently
• Apply knowledge of professional practice and regulatory requirements
• Demonstrate effective professional communication, teamwork, and conflict resolution skills
• Develop treatment protocols and home exercise programs for patients
• Educate patients and families on therapeutic techniques and safety
• Extract and apply evidence-based practice information to clinical care
• Perform assessments, evaluations, and reassessments across a wide range of diagnoses including orthopedic, neurological, and cardiopulmonary conditions
• Proficiency with adaptive devices, assistive devices, gait training, and mobility equipment required
• Maintain proficiency with required technology and computer systems
• 80 orientation hours required
