Physical Therapist Job in Lakeport, CA

Join Sutter Health Care at Home Lakeside as a Physical Therapist providing home health services to adult and geriatric patients in the Lakeport area. You will deliver skilled physical therapy in patients' homes, completing approximately 5 weighted visits per day with 4–5 patient contacts daily.

Role Responsibilities:

• Provide comprehensive physical therapy evaluations, reassessments, and treatment planning
• Develop short and long-term patient goals aligned with home health protocols
• Complete OASIS documentation and maintain accurate clinical records
• Deliver skilled interventions including gait training, orthopedic and neurological rehabilitation, and post-stroke recovery
• Create and educate patients on home exercise programs
• Assess and recommend adaptive and assistive devices, hoyer lifts, and specialty equipment

Required Qualifications:

• Valid California Physical Therapy license
• Minimum 1 year experience as a Home Health Physical Therapist
• Valid California driver's license and auto insurance
• Proficiency with gait training, OASIS documentation, orthopedic and neurological treatment
• Experience with adaptive devices and specialty equipment

Preferred Qualifications:

• BLS certification
• Experience with Parkinson's disease, Multiple Sclerosis, dementia, spinal cord injuries, and traumatic brain injuries
• Knowledge of Medicare Part A and B, Medicaid, and workers' compensation billing

Schedule and Details:

• 5 days per week, 8-hour days
• No call required; no float to other facilities
• 40 hours of orientation provided
• 13-week assignment starting August 10, 2026
• Weekly pay: $3,301
