PT - Physical Therapist

Job Details:

• Restore function to prevent or minimize disability following disease or injury
• Help clients reach maximum performance and function within the limits of their capabilities
• Evaluate and treat patients using medically prescribed physical therapy programs
• Supervise Physical Therapy Assistants as needed
• Conduct initial admission and evaluation visits
• Document observations, clinical findings, problems, skilled interventions and treatments, goals, and discharge plans
• Notify assigned Clinical Team Manager of OASIS scores within established timelines
• Travel to assigned territory including Brisbane, Daly City, Colma, South San Francisco, Pacifica, and neighboring cities as needed
• Minimum one year of Home Health experience required
