Physical Therapist Home Health Job in Sunnyvale, CA

Join Pathways Home Health & Hospice as a Physical Therapist serving patients across San Mateo County. You will deliver skilled physical therapy services in patients' homes, helping them restore function and achieve their maximum performance following disease or injury.

Your Responsibilities:

• Evaluate and treat patients using medically prescribed physical therapy programs
• Conduct initial admission and evaluation visits
• Restore function to prevent or minimize disability
• Document clinical findings, skilled interventions, treatment goals, and discharge plans
• Notify the Clinical Team Manager of OASIS scores within established timelines
• Supervise Physical Therapy Assistants as needed
• Travel to assigned territory including Brisbane, Daly City, Colma, South San Francisco, Pacifica, and neighboring cities

Position Details:

• Schedule: 5 days per week, 8-hour shifts
• Duration: 91 days
• Start Date: July 13, 2026
• Weekly Pay: $3,143

Qualifications:

• Degree in Therapy (Physical Therapist credential required)
• Minimum one year of home health experience
