Physical Therapist Job in Woodland, CA

Woodland Healthcare Home Health is seeking a Physical Therapist for home health services in the Yolo County area. You will work 8:30 AM to 5:00 PM, Monday through Friday, with typically one Saturday per month on a 91-day contract starting June 23, 2026.

About the Role

You will provide physical therapy services according to written physician plans of care. Your responsibilities include:

• Assess, evaluate, and update patients' therapeutic, rehabilitative, and functional status
• Evaluate home environments and assess muscle strength, mobility, gait, and range of motion
• Instruct patients and families in the use and care of therapeutic appliances and prosthetic devices
• Initiate physical therapy programs and train personnel and family members
• Supervise homemaker and Home Health Aides participating in patient rehabilitation
• Report patient reactions to treatment and condition changes to physicians
• Prepare and submit clinical and progress summaries
• Participate in peer consultation and staff competency assessments

Requirements

• Minimum 2 years of experience as a PT in acute, home care, or rehab settings
• BLS certification required

Details

Service area includes Yolo County territories: Woodland, Davis, Yolo, Dunnigan, Arbuckle, West Sacramento, Natomas, Knights Landing, Capay, Vacaville, Winters, and Esparto. Mileage is reimbursable per GSA guidelines with proper documentation. You will use the Homecare Homebase charting system.

Dress code allows scrubs of any color, or slacks (khakis acceptable) with a polo or button-up shirt.

Compensation

Weekly pay is $2,423.60.
