Physical Therapist Job in Novato, CA

ADEX Healthcare Staffing is seeking a Physical Therapist for a 91-day travel assignment with Sutter Health Care At Home Outer Bay Rowland Way in Novato, California. You will work 5 days per week on day shifts, completing approximately 5 patient visits daily across South Marin County territories including Sausalito, Tiburon, Belvedere, and Mill Valley.

Your Responsibilities:

• Provide physical therapy services to adult and geriatric patient populations in home health settings
• Perform patient evaluations, reassessments, and discharge planning
• Develop and implement individualized treatment protocols and home exercise programs
• Collaborate with interdisciplinary support teams including physical therapy and social services staff
• Provide patient and family education as part of comprehensive care delivery
• Utilize critical thinking and complex problem-solving skills to work independently in patients' homes
• Maintain professional communication and documentation standards
• Document care using OASIS and EPIC systems

Requirements:

• Valid California Physical Therapy license
• BLS certification (AHA or ARC)
• Valid driver's license and auto insurance
• EPIC system experience required
• OASIS documentation proficiency required
• Minimum 1 year of clinical experience
• Travel experience required
• Experience with orthopedic, neurological, and cardiopulmonary conditions preferred

This assignment begins August 3, 2026, and offers weekly compensation of $3,054. You will have the opportunity to deliver direct patient care while maintaining flexibility in a home-based therapy environment.
