Physical Therapist Job in Sunnyvale, CA

Pathways Home Health & Hospice is seeking a Physical Therapist to deliver skilled home health services across the greater Palo Alto area. You will work with patients in their homes to restore function, prevent disability, and help clients achieve their maximum performance within their capabilities.

Your Responsibilities:

• Evaluate and treat patients using medically prescribed physical therapy programs
• Perform initial admission and evaluation visits
• Supervise Physical Therapy Assistants as needed
• Document clinical findings, skilled interventions, treatment goals, and discharge plans
• Notify your Clinical Team Manager of OASIS scores within established timelines
• Serve patients across a defined territory including Palo Alto, Atherton, Cupertino, East Palo Alto, Los Altos, Los Altos Hills, Menlo Park, Mountain View, Portola Valley, Santa Clara, Stanford, Woodside, Sunnyvale, and neighboring cities as needed

Position Details:

• Schedule: 5 days per week, 8-hour shifts
• Duration: 91 days
• Start Date: September 8, 2026
• Weekly Pay: $2,920

Requirements:

• Minimum one year of home health experience required
