Physical Therapist Home Health Job in Novato, CA

Sutter Health Care At Home Outer Bay Rowland Way is seeking a Physical Therapist to deliver skilled rehabilitation services to adult and geriatric patients in the home health setting. This is a 91-day travel assignment beginning July 20, 2026.

Role Overview

You will manage an average daily census of 5 patients with approximately 5 visits per day. Your responsibilities include:

• Conducting physical therapy evaluations, reassessments, and functional capacity examinations
• Developing and implementing individualized treatment protocols and home exercise programs
• Providing patient and family education on therapy goals and safety
• Identifying environmental barriers and safety concerns within the home
• Collaborating with an interdisciplinary team including physical therapy and social services staff
• Documenting care using OASIS standards and the EPIC electronic health record system

Required Qualifications

• Valid California Physical Therapy license
• Minimum 1 year of clinical experience
• Current BLS certification (AHA or ARC)
• Valid driver's license and active auto insurance
• EPIC system experience
• OASIS documentation experience in home health settings
• Travel healthcare experience required

Key Competencies

You must demonstrate proficiency with adaptive devices, assistive devices, and specialized mobility equipment. Strong critical thinking and complex problem-solving skills are essential for managing multiple patient priorities independently in a demanding environment. Excellent verbal and written communication abilities are required for effective team collaboration and patient interaction. Charge experience is preferred.

Assignment Details

Schedule: 5 days per week, 8-hour days (Days shift). This 91-day position includes 80 hours of structured orientation to familiarize you with facility protocols, patient populations, and documentation systems.

Weekly pay: $2,812.32
