Pharmacist Inpatient Job in San Francisco, CA

You'll work as an inpatient pharmacist at Sutter Health CPMC Van Ness Campus in San Francisco. This is a Monday–Friday position with five 8-hour day shifts per week. There are no weekend, holiday, or on-call obligations.

Your responsibilities include:

• Providing clinical pharmacy services for a daily census of approximately 12 adult inpatients
• Managing inpatient and outpatient medication therapy across multiple therapeutic areas
• Performing clinical dosing calculations, including cardiac dosing, renal dosing, and pharmacokinetic dosing
• Administering and monitoring high-alert medications, anticoagulation therapy, and pain-controlled analgesia
• Preparing IV medications, IV piggyback doses, IV push medications, and TPN admixtures using proper sterile technique
• Monitoring therapeutic drug levels and adjusting therapy as needed
• Managing Warfarin dosing and antibiotic therapy
• Documenting clinical interventions and patient information in the Epic charting system

Required qualifications:

• PharmD degree and valid RPh licensure
• Minimum 2 years of pharmacist experience
• Documented inpatient and outpatient pharmacy experience
• Transplant pharmacy experience required
• Proficiency with Epic electronic health records
• Competency in ambulatory care, compounding, sterile technique, and clinical medication monitoring

This 91-day assignment begins November 2, 2026. Weekly pay is $3,262. You'll join a healthcare team serving the San Francisco area and gain experience in a busy inpatient setting with exposure to complex medication management and clinical decision-making.
