PT - Physical Therapist

Job Details:

• Home health physical therapy position requiring travel to patient homes
• Provide direct patient care and physical therapy services to home health clients
• Collaborate with other team members and participate in development of comprehensive patient care plans
• Work independently with minimal supervision or assistance
• Utilize rehabilitation equipment including splints, orthoses, prostheses, wheelchairs, ADL equipment, and modalities
• Rotating weekends and holiday coverage required as needed
• No on-call responsibilities
• Must have own vehicle for this need
• Valid driver's license required
• 2+ years of acute nursing care (hospital based) and home care experience required
• Certified home health experience including proficiency with OASIS required and non-negotiable
• Home Health Certification required
• Massachusetts Physical Therapist License required - must be uploaded with profile
• Active American Heart Association BLS certification required
• Skills checklist for Physical Therapist current to within 1 year required
• 1 supervisory signed reference from within the past two years of employment required
• Client prefers 45 minutes or less commute from tor office in Pittsfield, MA
