Job Details:

• 8-hour shifts with no on-call responsibilities
• No weekend shifts required unless a holiday falls within Monday through Friday, in which case hours may be made up on the weekend
• Missouri state license required
• Minimum 1 year of physical therapy experience required
• Long-term care or rehabilitation experience required
• New graduates accepted provided they have Level II clinical experience (Level I observation experience alone does not qualify)
• Screen, evaluate, and rehabilitate patients with physical functioning disorders and/or wound care needs
• Develop appropriate plans of care following all regulatory and clinical practice standards
• Utilize standardized assessments and evidence-based practice to support clinical interventions
• Administer rehabilitation procedures including manual techniques, ambulation, therapeutic exercises, modalities, use of supportive and assistive devices, and wound care
• Assess patient progress and adjust treatments accordingly in clinical records per regulatory and clinical practice requirements
• Evaluate treatment outcomes for effectiveness and modify services or make referrals as appropriate
• Provide consultation and counseling to patients, families, caregivers, and other service providers related to physical disorders
• Communicate effectively with supervisors and health team members regarding patient progress, barriers, and treatment plans
• Provide comprehensive discharge summaries per regulatory and clinical requirements
• Document all required information in the electronic documentation system and maintain patient records per company policy
• Accurately record treatment minutes for all services provided
• Participate in required meetings including patient care conferences, utilization review meetings, and rehabilitation meetings
• Supervise and support physical therapist assistants, rehabilitation aides, and/or students as needed
• Adhere to APTA and state-required practice acts, code of conduct, and ethics standards
