Physical Therapist Job in Lewistown, MO

Tide Health Group - Country Aire in Lewistown, MO is hiring a Physical Therapist for a long-term care setting. This is a temporary position running 91 days, beginning July 27, 2026.

Position Details

• Schedule: 5 days per week, 8-hour day shifts
• No on-call or weekend shifts required (except if a holiday falls Monday–Friday, in which case you may make up hours on the weekend)
• Weekly pay: $2,721

Your Role

You will screen, evaluate, and rehabilitate patients with physical functioning disorders and wound care needs. You'll develop individualized plans of care aligned with regulatory and clinical practice standards, using standardized assessments and evidence-based interventions. Your responsibilities include administering rehabilitation procedures—manual techniques, ambulation training, therapeutic exercises, modalities, assistive devices, and wound care—while documenting progress in the electronic health record system and recording all treatment minutes accurately.

You'll assess patient progress regularly, adjust treatments as needed, evaluate outcomes, and make referrals when appropriate. You'll also provide consultation and counseling to patients, families, caregivers, and other providers regarding physical disorders, and communicate effectively with your supervisor and care team about patient progress and barriers to recovery.

Additional duties include preparing comprehensive discharge summaries per regulatory requirements, participating in patient care conferences and utilization review meetings, and supervising physical therapist assistants, rehabilitation aides, and students as needed.

Requirements

• Valid Missouri state physical therapy license
• Minimum 1 year of physical therapy experience
• Long-term care or rehabilitation experience required
• New graduates accepted if they have completed Level II clinical experience (Level I observation alone does not qualify)
• Adherence to APTA practice acts, code of conduct, and ethics standards
