Physical Therapist Job in Lexington, Kentucky

VNA Health at Home in Lexington is seeking a Physical Therapist for home health services. This is a full-time travel position running 91 days, starting July 27, 2026.

Schedule and Compensation

• Monday through Friday, 8:00 AM to 5:00 PM (5x8 day shifts)
• Rotating weekend shifts, 8:00 AM to 5:00 PM
• Holiday rotation required
• Weekly pay: $2,641

Clinical Responsibilities

• Complete 6.5 productivity points daily, including Start of Care (SOC), Resumption of Care (ROC), recertifications, evaluations, and patient visits
• Deliver home health physical therapy to patients with diagnoses such as CHF, orthopedic conditions, and pneumonia
• Provide direct patient care in home settings

Requirements

• Current BLS (AHA) certification
• Prior home health experience required
• Proficiency with Homecare Homebase (HCHB) charting system
• New graduates not preferred

Support and Resources

• Two-week orientation with a dedicated work coach, including field visits
• Clinical manager or educator available 24/7 for emergencies
• Clinical manager or educator available 8:00 AM to 5:00 PM for routine support
• Phone and tablet provided

Dress Code

• Scrubs in any color or business casual attire

Cancellation Policy

• Facility may cancel up to 4 shifts per 13-week assignment, or 3 shifts for assignments under 13 weeks
