PT - Physical Therapist

Job Details:

• Work 8-hour day shifts Monday through Friday from 8:00 AM to 5:00 PM with rotating weekends (8:00 AM to 5:00 PM)
• Rotating holiday schedule required
• Complete 6.5 points per day including Start of Care (SOC), Resumption of Care (ROC), recertifications, evaluations, and daily visits
• Provide home health physical therapy services in a post-acute care setting
• Treat patients with common diagnoses including CHF, orthopedic conditions, and pneumonia
• Must have Home Care Home Base (HCHB) experience and home health experience
• Chart using Homecare Homebase (HCHB) charting system
• Phone and tablet will be provided for use in the field
• Clinical manager or educator available 24/7 for emergencies and 8:00 AM to 5:00 PM for general support
• 2-week orientation with a work coach including field visits
• Either scrubs or business casual attire acceptable
• Required certification: BLS (AHA)
• No new graduates preferred
