Job Details:

• 3rd shift position working 3x12-hour shifts from 7:00 PM to 7:00 AM
• Implement professional nursing practice centered on the patient as a whole person and creation of a caring environment focused on healing and wellness
• Base nursing practices on evidence, current research, hospital and department policies and procedures, regulatory standards, and professional guidelines
• Apply all four phases of the nursing process along with critical thinking and creativity in clinical decision-making
• Conduct nursing rounds, assessments, and treatments for direct patient care
• Facilitate admission, transfer, and discharge activities
• Provide patient and family-centered education and individualized treatment planning
• Perform and document comprehensive assessments in accordance with RN scope of practice
• Consider patient culture, age, and developmental level in all care decisions
• Identify patient risk factors and presenting complaints
• Collect objective and subjective data and obtain patient health information from family or others as necessary
• Develop and prioritize patient problem lists based on input from the inter-professional team
• Determine desired outcomes in coordination with patient and family in accordance with medical plan of care
• Explain procedures, treatments, and processes to patients and families
• Recognize, respond to, and communicate changes in patient condition appropriately
• Delegate tasks appropriately and oversee team members in the performance of patient care activities
• Monitor accuracy and completeness of delegated patient care provided by other team members
• Identify and address educational needs of patients related to prioritized problems
• Facilitate patient discharge process including discharge teaching and coordination with Case Management
• Actively participate in unit-based quality improvement activities and core measures including patient satisfaction, patient falls, pressure ulcers, and CAUTIs
• Promote a positive group dynamic and healing environment through words and actions
