Job Details:

• 3rd shift position working 3x12-hour shifts from 7:00 PM to 7:00 AM
• Weekends and holidays required as needed
• Implement professional nursing practice centered on the patient as a whole person, focusing on healing and wellness
• Perform and document comprehensive patient assessments in accordance with RN scope of practice
• Conduct nursing rounds, assessments, and treatments
• Facilitate admission, transfer, and discharge activities
• Provide patient and family-centered education and individualized treatment planning
• Identify patient risk factors, cultural considerations, age, and developmental level during assessments
• Collect objective and subjective data and verify initial admission assessments
• Develop and prioritize patient problem lists in coordination with the inter-professional team
• Determine desired outcomes in coordination with patients and families in accordance with the medical plan of care
• Explain procedures, treatments, and processes to patients and families
• Recognize, respond to, and communicate changes in patient condition appropriately
• Delegate tasks appropriately and oversee team members in the performance of patient care activities
• Monitor accuracy and completeness of delegated patient care provided by other team members
• Identify and address educational needs of patients related to prioritized problems
• Facilitate timely patient discharge process and coordinate with Case Management as indicated
• Actively participate in unit-based quality improvement activities including patient satisfaction, fall prevention, pressure ulcer prevention, and CAUTI monitoring
• Apply evidence-based practice, current research, and hospital policies and procedures to clinical decision-making
