Opportunity:
PT - Physical Therapist
Job Details:
• Provide physical therapy services within the legal scope of practice, following all licensure requirements
• Conduct initial evaluations, progress notes, 30-day reassessments, and 60-day recertifications in accordance with Medicare and state standards
• Develop, implement, and update individualized plans of care aligned with physician orders and patient needs
• Deliver skilled interventions and maintenance therapy programs as permitted by state-defined scope of practice
• Directly supervise Physical Therapist Assistants (PTAs) and Home Health Aides in compliance with state practice acts
• Consult with physicians and interdisciplinary team members regarding treatment adjustments and patient progress
• Educate patients and caregivers on home exercise programs, mobility, and adaptive strategies to promote independence and safety
• Prepare and submit OASIS assessments, recertifications, transfers, resumptions of care, and discharges within agency-defined timeframes
• Apply ICD-10 coding accurately for compliant documentation
• Present in-service programs periodically to agency staff and contribute to staff education initiatives
• Actively engage in Quality Assessment and Performance Improvement (QAPI) activities
• Document clinical interventions, care coordination, and communication in a timely and accurate manner
• Participate in interdisciplinary meetings, case reviews, and continuing education opportunities
• Work independently with minimal daily oversight within a home health setting
• See patients within a 45-mile radius of the Albuquerque area including Tijeras, Rio Rancho, Bernalillo, Bosque Farms, Los Lunas, and Belen
• Reliable transportation, valid driver's license, and auto insurance required for home visits
• Graduate of an accredited physical therapist curriculum required
• Current Physical Therapist licensure in the state of New Mexico required
• Two years of clinical experience preferred; home health experience a plus but not required
• Current CPR certification required
• Basic computer proficiency required; willingness to learn EMR systems and OASIS documentation
• Commitment to compliance with Medicare/Medicaid Conditions of Participation, state regulations, and agency policies
