Physical Therapist Job in New Lenox, IL

Silver Cross Hospital is seeking a Physical Therapist for a temporary position in home health settings throughout New Lenox, IL. This role offers you the opportunity to work directly with patients in their own environments, providing personalized care and rehabilitation services.

Position Details:

• Schedule: 5 days per week, 8-hour shifts (days)
• Duration: 91 days
• Start date: September 21, 2026
• Weekly pay: $2,581

Your Responsibilities:

• Evaluate physical therapy needs and establish individualized goals and treatment plans for assigned patients
• Assess, plan, treat, evaluate, and implement comprehensive plans of care
• Provide age and developmentally appropriate care to patients ranging from pediatric to geriatric populations
• Transfer and position patients as needed during treatment
• Complete timely documentation of all patient care activities in the electronic medical record system
• Demonstrate understanding of the admission process, treatment costs, coding, and appropriate billing practices
• Communicate and interact effectively with patients, families, physicians, and staff members

Required Qualifications:

• Illinois state physical therapy license, active and current at time of submission
• Minimum 1 year of home health PT experience
• Valid driver's license and current car insurance
• BLS certification (Basic Life Support)
• OASIS and start of care experience strongly preferred

Additional Information:

On-call and weekend requirements will be determined based on assignment needs. You will be working in a home health capacity, delivering direct patient care in residential settings while maintaining professional standards and documentation requirements.
