Job Details:

• No on-call or weekend requirements
• Minimum 2 years of physical therapy experience in a long-term care (LTC) setting required
• Texas state license or compact license required
• Accurately assess, differentially diagnose, and identify deficits in a patient's ability to move and perform functional activities in daily life
• Perform patient evaluations through obtaining history, performing relevant systems reviews, and selecting and administering specific tests and measures
• Develop individualized treatment plans with patient-centered goals to reduce pain, restore function, and prevent disability
• Establish and select treatment techniques that address specific impairments identified during evaluation and in accordance with physician orders
• Conduct ongoing reassessment of patients and their response to treatment
• Initiate and manage discharge planning beginning at first visit and continuing throughout treatment
• Supervise and delegate to ancillary staff and students consistent with state laws and department policy
• Complete accurate and clear documentation within established timeframes and in accordance with accreditation requirements and insurance regulations
• Communicate with patients, families, caregivers, and members of the healthcare team to promote maximum benefit of care
• Assess and provide education to patients, families, and caregivers to meet their educational needs
• Participate in professional growth and development through educational programs, in-service meetings, workshops, and current literature review
• Actively participate in quality improvement activities
• Maintain a safe environment for patients, visitors, and staff in compliance with approved safety standards
• Function within scope of practice as defined by the state's practice act
