Physical Therapist Job in Missoula, MT

Partners in Home Care Inc. is seeking a Physical Therapist to provide skilled home health services to patients throughout the Missoula, MT service area.

Schedule and Pay

• Monday through Friday, 8:00 AM to 5:00 PM (occasional evening and weekend coverage as needed)
• 5x8 Days shift
• $3,060 weekly pay
• 84-day travel assignment beginning October 6, 2026

Role Responsibilities

• Evaluate patients and develop individualized plans of care tailored to the home environment
• Provide direct patient care according to physician orders and established care plans
• Apply evidence-based physical therapy practice guidelines and Medicare regulations in evaluations and treatment
• Modify treatment plans as appropriate, adjusting frequency and duration of care
• Complete CMS-required OASIS scoring and collaborate with the Quality team on necessary adjustments
• Perform skilled interventions within the physical therapy practice act scope, including wound care when applicable
• Participate in multidisciplinary conferences to establish appropriate patient care plans
• Maintain point-of-care documentation with accurate records of skilled services, adverse events, and relevant communications
• Track accurate time logs and mileage records per agency policy
• Oversee services provided by Physical Therapist Assistants and complete required supervisory visits
• Supervise CNAs when the RN is no longer involved in the care plan
• Participate in Quality Assurance and Performance Improvement initiatives

Requirements

• BS degree or higher from an accredited Physical Therapy program
• Current Montana Physical Therapy license
• Valid driver's license, reliable transportation, and appropriate auto insurance
• Minimum 2 years of previous physical therapy experience preferred
• Maintenance of physical therapy licensure requirements and continuing education standards
