Job Details:

• Graduate degree from an accredited Physical Therapy Program approved by the APTA required
• Minimum of one year of physical therapy experience required; home health or facility experience preferred
• Current and unrestricted Physical Therapy licensure required
• Current CPR certification required
• HomeCare HomeBase charting experience preferred
• Travel experience required
• Two supervisory references required
• Keep requested time off to a minimum; preferably no requests for a full calendar week off during assignment
• Any requested time off must be included in initial submission and approved in advance of offer
• Expected to work some holidays
