Pharmacist Outpatient Job in Fort Worth, TX

JPS Healing Wings & Infectious Disease Clinic seeks an Allied Health Professional Pharmacist for outpatient pharmaceutical care in our Fort Worth, Texas location. This is a full-time onsite role running 91 days, starting October 20, 2026.

Schedule and Compensation

• Monday through Friday, 8:00 AM to 5:00 PM (40 hours per week)
• $3,820 weekly pay

Role Responsibilities

• Provide clinical pharmaceutical care as a mid-level provider, seeing approximately 14 patients daily
• Deliver clinical leadership for pharmaceutical care within assigned areas
• Serve as a clinical resource for outpatient and inpatient clinicians on HIV and Hepatitis pharmacotherapy
• Collaborate with the Inpatient Infectious Diseases Clinical Pharmacist to manage patient transitions from hospital to the HIV Clinic, ensuring continuity of appropriate therapy
• Develop and optimize pharmacotherapeutic regimens through collaborative practice agreements
• Conduct drug utilization reviews and monitor for drug interactions and adverse events
• Work to minimize polypharmacy and develop individualized patient medication action plans focused on self-management goals
• Serve as preceptor for pharmacy students and residents; participate in didactic and experiential education
• Function as a key core multidisciplinary team member for patient care within the Medical Home model

Required Qualifications

• Doctorate of Pharmacy or equivalent from an accredited College of Pharmacy
• 2-year American Society of Health System Pharmacists (ASHP) residency in HIV
• 5 years of direct HIV patient care with demonstrated medication management expertise
• Active pharmacy license from the Texas State Board of Pharmacy or eligibility for licensure
• BLS certification
