PT - Physical Therapist

Job Details:

• Home Health Physical Therapist position
• Day shift schedule, Monday through Friday from 8:30 AM to 5:00 PM
• 5x8-hour shifts per week
• Guaranteed hours based on hours signed
• No on-call required
• Clinician may float within scope to meet facility needs
• Mileage reimbursement provided for travel between patient locations
• Block scheduling not available
• Minimum 1 year of experience required; 2 years preferred
• Must have PT Home Health experience
• Serves patient age groups including adolescents, adults, and geriatrics
• Daily census of 6 units
• Pending state license accepted but must be obtained prior to starting
• Required certifications: BLS, PT State License
• All requested time off must be included on initial submission; time off requests after the fact will not be considered
• Attendance policy in effect: missing more than two shifts per 13-week contract without reasonable explanation may result in contract cancellation
