Opportunity:

RN - Case Management

Job Details:

• Work 8-hour day shifts, five days a week
• Expected to work every other weekend and holidays in rotation with hospital employees
• Night shift not applicable for this role
• Patient ratios: 1:25 on floors and 1:20 in UC units
• Work with equipment including iPad, computer, and cell phone
• Document in Midas and review in Meditech; utilize Careport platform for referral process
• Perform assessments and evaluations
• Conduct discharge planning with patients and families
• Facilitate and lead multidisciplinary rounds (MDR) discussions
• Hospital and/or case management experience required
• Experience with Midas, Interqual, Meditech, and acute hospital case management preferred
• Must be independent, proactive, and autonomous with strong critical thinking and troubleshooting skills
• Scrub color: Navy Blue
• Parking available anywhere except the ER parking area
