Physical Therapist Job in Lewistown, MT

Central Montana Med Ctr is seeking a Physical Therapist to deliver home health physical therapy services to patients enrolled in our Home Health and Hospice programs. You will work on a days shift and travel to patient homes and other care settings throughout the Lewistown area.

Your responsibilities will include:

• Receive and review physician therapy orders for assigned patients
• Complete comprehensive patient evaluations and establish written treatment plans with short-term and long-long-term goals
• Provide skilled physical therapy interventions including therapeutic exercises, stretching, mobility training, balance activities, pain management techniques, and functional rehabilitation
• Assess home safety and recommend equipment, assistive devices, or environmental modifications to promote independence and prevent falls
• Monitor patient progress and modify treatment plans as needed to achieve optimal outcomes
• Educate patients, caregivers, and family members regarding treatment plans, home exercise programs, safety measures, and expected outcomes
• Maintain accurate and timely clinical documentation including evaluations, progress notes, discharge summaries, recertifications, resumptions of care, and discharge documentation
• Collaborate and communicate effectively with physicians, nurses, therapists, and other healthcare professionals involved in patient care

Required qualifications:

• Current Physical Therapist licensure in the state of Montana
• Valid driver's license
• BLS certification

This is a 91-day assignment with a weekly pay rate of $2,458. Your start date is October 26, 2026.
