About this role

Santa Rosa Post Acute is seeking a Physical Therapist for a 91-day travel assignment beginning September 21, 2026. You will deliver direct patient care in a post-acute setting, where your expertise in rehabilitation and functional recovery makes a real difference.

Your responsibilities will include:

  • Performing comprehensive physical evaluations to assess patients' functional abilities and limitations
  • Developing and implementing individualized treatment plans focused on improving strength, mobility, and independence
  • Providing hands-on physical therapy interventions using evidence-based techniques and therapeutic modalities
  • Instructing patients and families on therapeutic exercises, proper body mechanics, and home safety modifications
  • Coordinating care with occupational therapists, nurses, physicians, and other team members to optimize patient outcomes
  • Monitoring progress regularly and modifying treatment plans based on patient response and functional goals
  • Documenting assessments, progress notes, and treatment plans accurately and timely per facility standards
  • Ensuring all care aligns with regulatory requirements and physical therapy best practices

Required qualifications:

  • Bachelor's, Master's, or Doctoral degree in Physical Therapy from an accredited program
  • Valid California Physical Therapy license
  • Knowledge of physical therapy techniques, therapeutic modalities, and adaptive equipment
  • Ability to work independently and collaboratively within an interdisciplinary team

Preferred experience:

  • Prior work in a skilled nursing facility or similar post-acute healthcare setting
