Case Management RN Job in Juneau, Alaska

Juneau SE Medical Primary Care Clinic is seeking a registered nurse for a Case Management role focused on women's health services. You'll work the night shift and contribute to comprehensive care coordination for patients across the lifespan.

Your responsibilities will include:

• Provide professional nursing care in accordance with organizational standards, policies, and evidence-based practice
• Deliver culturally sensitive, patient-centered services and conduct comprehensive patient assessments
• Develop individualized plans of care and administer medications and treatments safely per provider orders
• Recognize changes in patient condition and initiate appropriate interventions or escalation of care
• Provide comprehensive care management for breast and cervical cancer prevention, screening, and diagnostic services
• Collaborate with providers, radiology, and interdisciplinary teams to promote optimal patient outcomes and safety
• Provide patient and family education related to care, medications, and discharge needs
• Facilitate referrals, follow-up care, and continuity of services
• Document assessments, interventions, and outcomes accurately and timely in the electronic health record
• Advocate for patients and assist them in navigating the healthcare system
• Float to other departments or assignments as needed

Requirements:

• Active Alaska nursing license at time of submission
• Associate's degree in Nursing from an accredited program
• Basic Life Support (BLS) certification
• Case Management certification required within 2 years of hire
• Proficiency with electronic health records and clinical systems
• Ability to lift, push, pull, and move up to 50 pounds
• Ability to stand for extended periods and perform bending, stooping, and kneeling

This is a 91-day travel assignment starting November 23, 2026. Weekly pay is $2,940.
