Physical Therapist Job in San Ramon, CA

CVH Care Home Health is seeking a Physical Therapist to provide therapy services in a home health setting across assigned county territories in the San Francisco Bay Area. You will work on a days shift for a 91-week assignment beginning October 12, 2026.

Service Areas

• Area 1: Fremont, Castro Valley, Ashland, Cherryland, Hayward, Newark, San Leandro, San Lorenzo, and Union City
• Area 2: Oakland, Berkeley, Alameda, Albany, Emeryville, and Piedmont
• Travel outside assigned areas and weekend coverage may be required

Compensation and Scheduling

You will earn $2,190 weekly. Two options are available: a guarantee option requiring minimum productivity of 27 points per week with a 36-hour guarantee, or a non-guarantee option with per-visit payment and no guaranteed hours. The guarantee does not apply during the first and last weeks of your assignment, weeks with holidays, or weeks with approved time off.

Visit Types and Productivity

You will manage various visit types including OASIS ReCertification, OASIS RoC, Routine, Eval, Discharge, Eval SoC, Eval RoC, Eval ReCert, Eval F/up, Re-Eval, Eval Discharge, OASIS Discharge, and OASIS F/up. Productivity is calculated as 1 point equals 1.33 hours, with OASIS SoC valued at 1.5 points or 2 hours. The rate is all-inclusive and covers drive time, travel time, visit time, documentation time, and phone calls with patients and physicians.

If you choose the guarantee option, you must be willing to travel outside the service area or work weekends to maintain weekly visit expectations.
