PT - Physical Therapist Outpatient

Job Details:

• Full-time outpatient physical therapist position with potential inpatient coverage as needed
• Assess, plan, organize, and participate in rehabilitative programs to improve patient mobility, relieve pain, increase strength, and improve or correct disabling conditions resulting from disease or injury
• Develop and update individualized patient Plans of Care (POC) based on evaluation findings and diagnosis
• Complete and document patient evaluations and tests per established standards and regulatory requirements
• Coordinate patient care effectively and efficiently with other care providers and support team members
• Provide clinical supervision to assistants and/or aides
• Serve as clinical instructor for assigned students
• Perform general clerical tasks including answering phones, updating patient appointments, and managing electronic health records (EHR)
• Participate in quality improvement processes and interdisciplinary projects
• Maintain a clean, neat, and safe working environment, including cleaning and disinfecting equipment after each use
• Works flexible shift assignments including variable days of the week and hours
• Minimum 1 year of experience required
• Epic charting system experience preferred
• Graduate of an accredited physical therapy graduate program required
• Required certifications: CA Physical Therapist License, BLS Certification
