Physical Therapist Job in Purcell, OK

Tide Health Group - Purcell Care Center is seeking a Physical Therapist for a long-term care position. You will work day shift, 5 days per week, 8 hours per day, with no on-call or weekend requirements.

About the Role

You will screen, evaluate, and rehabilitate patients with physical functioning disorders and wound care needs. Your responsibilities include:

• Develop and implement treatment plans following regulatory and clinical practice standards
• Use standardized assessments and evidence-based practice to guide clinical interventions
• Administer rehabilitation procedures including manual techniques, ambulation, therapeutic exercises, modalities, assistive devices, and wound care
• Assess patient progress and adjust treatments in the electronic health record per regulatory requirements
• Evaluate treatment outcomes and modify services based on patient response
• Provide consultation and counseling to patients, families, caregivers, and other service providers
• Communicate with supervisors and team members regarding patient progress, barriers, and treatment plans
• Document treatment minutes and clinical information accurately per all regulatory requirements
• Participate in patient care conferences, utilization review meetings, and rehabilitation meetings
• Supervise and support physical therapist assistants, rehabilitation aides, and students
• Provide comprehensive discharge summaries meeting regulatory standards

Requirements

• Current Oklahoma state license as a Physical Therapist
• Minimum 1 year of physical therapy experience
• Long-term care or rehabilitation experience required
• New graduates accepted with Level II clinical experience
• Adherence to APTA and state-required practice acts, code of conduct, and professional ethics

Assignment Details

This is a 91-day temporary assignment beginning July 27, 2026. You will earn $2,448 per week.
