Physical Therapist Job in Moore, OK

Tide Health Group - Thunder Care and Rehab in Moore, OK is seeking a Physical Therapist for a long-term care position. This is a temporary role running 91 days, starting July 27, 2026.

Role Overview

You will screen, evaluate, and rehabilitate patients with physical functioning disorders and wound care needs. You'll develop plans of care that follow all regulatory and clinical practice standards, using standardized assessments and evidence-based practice to guide your clinical interventions.

Your Responsibilities

• Administer rehabilitation procedures including manual techniques, ambulation, therapeutic exercises, modalities, and use of supportive and assistive devices
• Assess patient progress and adjust treatments according to regulatory and clinical requirements
• Evaluate treatment outcomes for effectiveness and modify services based on patient evaluation
• Provide consultation and counseling to patients, families, caregivers, and service providers
• Communicate effectively with supervisors and health team members regarding patient progress and treatment plans
• Provide comprehensive discharge summaries per regulatory requirements
• Document all required information in the electronic documentation system and maintain patient records
• Accurately record treatment minutes for services provided
• Participate in patient care conferences, utilization review meetings, and rehabilitation meetings
• Supervise and support physical therapist assistants, rehabilitation aides, and students

Schedule and Compensation

This position offers a 5x8 day shift schedule with no on-call or weekend requirements. You will earn $2,731 per week.

Requirements

• Oklahoma state license
• Minimum 1 year of physical therapy experience
• Long-term care or rehabilitation experience required
• New graduates accepted with Level II clinical experience
• Adherence to APTA and state-required practice acts, code of conduct, and ethics
