Physical Therapist Job in Purcell, OK

Tide Health Group - Purcell Care Center is hiring a Physical Therapist for a long-term care setting in Purcell, Oklahoma. This is a temporary position working days, 5 days per week, 8 hours per shift. You'll earn $2,641 weekly, with an assignment duration of 91 days starting July 27, 2026.

Role Overview

You will screen, evaluate, and rehabilitate patients with physical functioning disorders and wound care needs. Your responsibilities include developing and implementing appropriate plans of care that follow regulatory and clinical practice standards. You'll utilize standardized assessments and evidence-based practice to support your clinical interventions.

Key Responsibilities

• Administer rehabilitation procedures including manual techniques, ambulation, therapeutic exercises, modalities, and use of supportive and assistive devices
• Assess patient progress and adjust treatments in the clinical record per regulatory requirements
• Evaluate treatment outcomes for effectiveness and modify services based on patient evaluation
• Provide consultation and counseling to patients, families, caregivers, and other service providers
• Communicate with supervisors and health team members regarding patient progress, barriers, and treatment plans
• Provide comprehensive discharge summaries per regulatory and clinical requirements
• Document all required information in the electronic documentation system and maintain patient records per company policy
• Accurately record treatment minutes for services provided
• Participate in patient care conferences, utilization review meetings, and rehabilitation meetings
• Supervise and support physical therapist assistants, rehabilitation aides, and students

Requirements

• Oklahoma state license required
• Minimum 1 year of physical therapy experience
• New graduates accepted with Level II clinical experience
• Long-term care or rehabilitation experience required
• Adherence to APTA and state-required practice acts, code of conduct, and ethics

No on-call or weekend requirements for this position.
