About this role

Tide Health Group - Ballard Nursing Center is seeking a Physical Therapist for a long-term care position in Ada, Oklahoma. This is a temporary role working day shifts, 5 days per week, 8 hours per day, for a 91-day assignment beginning July 27, 2026.

Role Responsibilities:

  • Screen, evaluate, and rehabilitate patients with physical functioning disorders and wound care needs
  • Develop and implement plans of care following regulatory and clinical practice standards
  • Administer manual techniques, ambulation training, therapeutic exercises, modalities, assistive devices, and wound care
  • Assess patient progress and adjust treatments accordingly in the clinical record
  • Evaluate treatment outcomes and modify services based on effectiveness
  • Provide consultation and counseling to patients, families, caregivers, and service providers
  • Communicate with supervisors and health team members regarding patient progress and treatment barriers
  • Provide comprehensive discharge summaries per regulatory requirements
  • Document all information in the electronic documentation system and maintain accurate patient records
  • Record treatment minutes for services provided per regulatory standards
  • Participate in patient care conferences, utilization review meetings, and rehabilitation meetings
  • Attend required facility and company trainings, in-services, and conferences
  • Obtain necessary DME or supplies per regulatory and clinical practice requirements
  • Supervise and support physical therapist assistants, rehabilitation aides, and students

Requirements:

  • Minimum 1 year of physical therapy experience
  • New graduates accepted with Level II clinical experience (Level I observation experience alone is insufficient)
  • Long-term care or rehabilitation experience required
  • Oklahoma state license required
  • Adherence to APTA and state-required practice acts, code of conduct, and ethics standards
  • No on-call or weekend requirements

Compensation: $2,731 per week.
