About this role

BayCare HomeCare Inc. is seeking a Physical Therapist for home health services in Sarasota and Charlotte County, Florida. This is a 140-week travel assignment running 5 days per week, 8 hours per day, with weekly pay of $2,474. The position begins October 5, 2026.

Role Responsibilities

  • Conduct 6–8 skilled home care visits daily for patients with Medicare, Medicare Advantage, and commercial insurance coverage
  • Treat adult and geriatric patients across multiple diagnoses: orthopedic, cardiac, post-surgical, neurological, COPD, CHF, stroke (CVA), falls, Parkinson's disease, amputation (AKA/BKA), respiratory conditions, and activities of daily living (ADL) support
  • Develop and implement home exercise programs tailored to patient needs
  • Perform patient and family education, safety assessment, and injury prevention planning
  • Manage documentation using BayCare Forms, BayCare Scheduling, and Ace Timesheet systems
  • Work 1–2 weekends during the contract period

Required Qualifications

  • Current Florida state license or compact license
  • Minimum 1 year of clinical experience
  • Valid driver's license and personal vehicle for home visit transportation
  • BLS certification (required)
  • Proficiency with adaptive equipment: gait belts, rolling walkers, sit-to-stand lifts, sliding boards, collars, standard walkers, supplemental oxygen, upper extremity prosthetics, and wheelchair assessment
  • Experience in orthopedic, neurological, and home health settings
  • OASIS training preferred; Cerner charting experience preferred

Essential Skills

  • Patient assessment and cardiopulmonary/circulatory evaluation
  • Clinical documentation and billing knowledge
  • Strong communication and patient education abilities
  • Proficiency with laptop, MiFi internet access, and portable printers

Dress code: solid color scrubs. This role requires clinical independence, strong assessment skills, and the ability to deliver quality rehabilitation services in the home environment.
