PT - Physical Therapist

Job Details:

• Conduct skilled home health visits with a minimum of 6 visits per day (minimum of 6 points per day, may increase by 1 point at manager's discretion)
• Work with patients including adult and geriatric populations with diagnoses such as med/surg, cardiac, ortho, hips/knees/shoulders, post-surgical, COPD, CHF, pneumonia, CVA, falls, weakness, Parkinson's, failure to thrive, respiratory conditions, neuro, trachs, trauma, AKA, BKA, and ADLs
• Self-schedule visits and hours based on the needs of the HomeCare office
• 1-2 weekend shifts required per contract
• Block scheduling type
• Required equipment: laptop, MiFi for internet access, and portable printer provided
• Use BayCare Forms, BayCare Scheduling, and Ace Timesheet documentation system for charting
• Dress code: any solid color scrubs
• Must have a valid driver's license and personal vehicle to conduct home visits
• 1 year of experience required; home health experience required
• OASIS training preferred
• Community hospital experience preferred; LTAC experience preferred
• Cerner charting experience preferred
• Required certifications: BLS, valid state license (FL license or compact license required)
• Required skills: assessment, cardiopulmonary/circulatory, documentation, use of adaptive and assistive devices (gait belt, rolling walker, sit-to-stand lift, sliding boards, soft/hard collars, standard walker, supplemental oxygen, upper extremity prosthetics, wheelchair assessment), orthopedics, neurology, home exercise program, medication management, patient and family education, recognition of signs and symptoms of physical/sexual/psychological abuse, safety and injury prevention, and billing
• Interdisciplinary support available including physical therapy, interpretation services, phlebotomy, social services, and pharmacy
