Physical Therapist Job in San Ramon, CA

You will provide physical therapy services to patients in a home health setting across a defined county territory. Your assigned service areas include Oakland, Berkeley, Alameda, Albany, Emeryville, Piedmont, Richmond, Rodeo, El Cerrito, El Sobrante, Hercules, Pinole, San Pablo, Crockett, and Kensington. Additional travel outside assigned areas may be required, and weekend coverage may be necessary to meet service expectations.

Position Options

• Guarantee option: Minimum 27 productivity points per week with 36 hours guaranteed. You must be willing to travel outside your service area or work weekends as needed to maintain weekly visit expectations.
• Non-guarantee option: Payment made per visit with no guaranteed hours. You may decline patient assignments without penalty.

Visit Types

Your caseload includes OASIS ReCertification, OASIS RoC, Routine, Eval, Discharge, Eval SoC, Eval RoC, Eval ReCert, Eval F/up, Re-Eval, Eval Discharge, OASIS Discharge, and OASIS F/up visits.

Compensation and Schedule

You will earn $2,190 per week working day shifts. The assignment runs for 91 days, beginning October 12, 2026. Your weekly rate covers drive and travel time, visit time, documentation time, and phone calls to patients and physicians.

Required Credentials and Documentation

• Current California Physical Therapist License
• BLS certification
• Valid Driver's License, Auto Insurance, and Auto Registration
• TB screening (annual)
• MMR, Varicella, and HepB immunizations
• TDap (within 10 years)
• Flu vaccination (annual)
• Drug screen and physical exam

You will work as an independent contractor in the home health setting, managing your own schedule within the framework of patient visit requirements and service area coverage.
