Job Details:

• Provide physical therapy services to patients in a home health setting
• Assigned county territory includes Area #2: Oakland, Berkeley, Alameda, Albany, Emeryville, Piedmont and Area #8: Richmond, Rodeo, El Cerrito, El Sobrante, Hercules, Pinole, San Pablo, Crockett, and Kensington
• Additional travel outside assigned areas may be required
• Weekend coverage may be required
• Guarantee option requires minimum productivity of 27 points per week with 36 hours guaranteed
• Must be willing to travel outside service area or work weekends if required to maintain weekly visit expectations under guarantee option
• Non-guarantee option allows patient refusal with no guaranteed hours and payment made on a per visit basis
• Visit types include OASIS ReCertification, OASIS RoC, Routine, Eval, Discharge, Eval SoC, Eval RoC, Eval ReCert, Eval F/up, Re-Eval, Eval Discharge, OASIS Discharge, and OASIS F/up
• Rate covers drive/travel time, visit time, documentation time, and time spent making phone calls to patients and doctors
• Required credentials: Current CA PT License, BLS, Driver's License, Auto Insurance, Auto Registration, TB (annual), MMR, Varicella, HepB, TDap (within 10 years), Flu (annual), Drug Screen, and Physical
