Tide Health Group - Ada Care Center is seeking a Physical Therapist for a long-term care position in Ada, Oklahoma. This is a temporary role running for 91 days, beginning July 27, 2026.

Position Details

  • Schedule: 5 days per week, 8-hour days (day shift)
  • Weekly pay: $2,534.40
  • No on-call or weekend requirements

Qualifications

  • Oklahoma state physical therapy license required
  • Minimum 1 year of physical therapy experience, or new graduates with Level II clinical experience
  • Long-term care or rehabilitation experience required

Key Responsibilities

  • Screen, evaluate, and develop rehabilitation plans for patients with physical functioning disorders and wound care needs
  • Administer rehabilitation procedures including manual techniques, therapeutic exercises, modalities, assistive devices, and wound care management
  • Assess patient progress and adjust treatments in accordance with regulatory and clinical standards
  • Evaluate treatment outcomes and modify services based on patient response
  • Provide consultation and counseling to patients, families, caregivers, and interdisciplinary team members
  • Communicate with supervisors and team members regarding patient progress, barriers, and treatment plans
  • Document all clinical information in the electronic system per company policy and regulatory requirements
  • Accurately record treatment minutes for all services provided
  • Prepare comprehensive discharge summaries
  • Participate in patient care conferences, utilization review meetings, and rehabilitation meetings
  • Supervise and support physical therapist assistants, rehabilitation aides, and students
  • Maintain compliance with APTA practice acts, code of conduct, and professional ethics standards
