Job Details:

• Provide physical therapy services to patients in a home health setting
• Assigned county territory includes Area #8 (Richmond, Rodeo, El Cerrito, El Sobrante, Hercules, Pinole, San Pablo, Crockett, Kensington) and either Area #2 (Oakland, Berkeley, Alameda, Albany, Emeryville, Piedmont) or Area #7 (Vallejo, Benicia, Napa, American Canyon, Yountville)
• Additional travel outside assigned area may be required
• Weekend coverage may be required
• Guarantee option requires minimum 27 points per week with 36 hours guaranteed
• Must be willing to travel outside the service area or work weekends if required to maintain weekly visit expectations under guarantee option
• Guarantee does not apply for the first and last week of assignment, weeks with holidays, or weeks with approved time off
• Non-guarantee option allows patient refusal with no guaranteed hours and payment made on a per visit basis
• Visit types and point conversions: OASIS ReCertification, OASIS RoC, Routine, Eval, Discharge, and related visit types = 1 point (1.33 hours); OASIS SoC = 1.5 points (2 hours)
• Rate is all inclusive and covers drive/travel time, visit time, documentation time, and time spent making phone calls to patients and doctors
• Required credentials: Current CA PT License, BLS, Driver's License, auto insurance, auto registration
