Physical Therapist Job in Rancho Cordova, CA

You'll deliver physical therapy services in a home health care setting across Rancho Cordova, treating patients in their homes, assisted living facilities, senior apartments, and board and care homes. This is a 91-day travel assignment running Monday through Friday, 8:30 AM to 5:00 PM, at $2,657 per week.

What you'll do:

• See 3 to 5 patients per day, which may include up to 2 Start of Care visits or 3 to 4 evaluations, depending on visit type
• Conduct Start of Care visits (weighted at 3.75 to 4 hours to account for travel, documentation, and case management)
• Treat orthopedic, cardiology, pulmonary, oncology, neurology, geriatric, and amputation cases—no pediatrics or hospice
• Supervise up to 2 PTAs or COTAs
• Work across service territories grouped to minimize travel and maintain case continuity
• Participate in 1 weekend per month (requested but not required)
• Work holidays with flexibility if time off is requested in advance

What you'll need:

• Valid California PT license
• 2+ years of healthcare experience (home health or community health preferred)
• Home health experience and OASIS knowledge required
• Current driver's license and proof of automobile insurance
• BLS (AHA) certification and NPI number
• Familiarity with Homecare Homebase (HCHB) charting preferred
• Strong oral, written, and organizational communication skills

Environment: You'll work in home settings that may be cramped, hot, cold, or cluttered. Dress code is business professional; scrubs are recommended but not provided. Closed-toe shoes required; no jeans, tank tops, or logo shirts. A 3-week orientation includes online learning and field precepting, with solo work expected by the end of week 2 or 3. Mileage is billable per GSA guidelines after your first assignment; travel from housing to your first visit does not qualify for reimbursement.
