Job Details:

• Provide physical therapy services in a home health setting across assigned county territories
• Assigned territory includes Area #4: Danville, Martinez, Moraga, Alamo, Clayton, Concord, Lafayette, Orinda, Pleasant Hill, and Walnut Creek
• Assigned territory also includes Area #5: Pittsburg, Discovery Bay, Byron, Antioch, Brentwood, Oakley, Bethel Island, and Bay Point
• Additional travel outside of assigned areas may be required
• Weekend coverage may be required
• Guarantee option requires a minimum of 27 points per week with a 36-hour guarantee
• Non-guarantee option is payment on a per visit basis with no guaranteed hours
• Visit types include OASIS ReCertification, OASIS RoC, Routine, Eval, Discharge, Eval SoC, Eval RoC, Eval ReCert, Eval F/up, Re-Eval, Eval Discharge, OASIS Discharge, and OASIS F/up
• 1 point equals 1.33 hours and OASIS SoC equals 1.5 points or 2 hours
• Rate covers drive and travel time, visit time, documentation time, and time spent making phone calls to patients and doctors
• Required credentials: Current CA PT License, BLS, Driver's License, auto insurance, and auto registration
