Physical Therapist Job in San Ramon, CA

CVH Care Home Health is seeking a Physical Therapist to deliver home health services across Contra Costa County. You'll work in a day shift role serving two assigned territories while building direct patient relationships in their homes.

Your Territories

• Area #4: Danville, Martinez, Moraga, Alamo, Clayton, Concord, Lafayette, Orinda, Pleasant Hill, and Walnut Creek
• Area #5: Pittsburg, Discovery Bay, Byron, Antioch, Brentwood, Oakley, Bethel Island, and Bay Point
• Additional travel outside assigned areas may be required

Your Responsibilities

• Provide physical therapy services across assigned county territories
• Conduct visit types including OASIS ReCertification, RoC, Routine, Eval, Discharge, Eval SoC, Eval RoC, Eval ReCert, Eval F/up, Re-Eval, Eval Discharge, OASIS Discharge, and OASIS F/up
• Complete documentation and communicate with patients and physicians
• Weekend coverage may be required

Compensation Structure

Weekly pay is $2,099. You may choose between a guarantee option or non-guarantee option. The guarantee option requires a minimum of 27 points per week with a 36-hour guarantee. The non-guarantee option is payment on a per-visit basis with no guaranteed hours. One point equals 1.33 hours; OASIS SoC equals 1.5 points or 2 hours. Your rate covers drive and travel time, visit time, documentation time, and time spent on patient and physician phone calls.

Required Credentials

• Current California PT License
• BLS Certification
• Valid Driver's License
• Auto insurance
• Auto registration

This assignment is 91 days, beginning October 12, 2026. You must have your own reliable transportation and maintain required auto documentation throughout your tenure.
