PT - Physical Therapist

Job Details:

• Conduct 5 patient visits per day within the South Marin County territory, including towns such as Sausalito, Tiburon, Belvedere, and Mill Valley
• Provide home health physical therapy services to adult and geriatric patient populations
• Complete OASIS assessments and documentation using EPIC electronic health records system
• Collaborate with interdisciplinary support teams including Physical Therapy and Social Services
• Develop and implement individualized treatment plans, home exercise programs, and treatment protocols
• Perform evaluations, reassessments, and develop short-term and long-term patient goals
• Apply critical thinking and complex problem-solving skills while working independently in a home health setting
• Identify and address environmental barriers within patients' home environments
• Provide patient and family education as needed
• Valid California PT license required
• BLS certification required (AHA or ARC)
• Valid driver's license and auto insurance required
• EPIC experience and OASIS experience required
• Minimum 1 year of experience required
• Travel experience required
