Tide Health Group - Thunder Care and Rehab is seeking a Physical Therapist for a long-term care and rehabilitation setting in Moore, Oklahoma. This is a temporary position running for 91 days, starting July 27, 2026.

Position Details

  • Schedule: 5 days per week, 8-hour shifts, days only
  • Weekly pay: $2,448
  • No on-call or weekend requirements

Qualifications

  • Oklahoma state physical therapy license required
  • Minimum 1 year of physical therapy experience, or new graduate with completed Level II clinical experience (Level I observation alone does not meet requirements)
  • Long-term care or rehabilitation experience required

Responsibilities

You will screen, evaluate, and rehabilitate patients with physical functioning disorders and wound care needs. You'll develop individualized plans of care aligned with regulatory and clinical standards, utilizing standardized assessments and evidence-based interventions. Your clinical work will include manual techniques, ambulation training, therapeutic exercises, modalities, assistive devices, and wound care management.

You'll assess patient progress regularly, adjust treatments in the clinical record, and evaluate outcomes for effectiveness. You will provide consultation and counseling to patients, families, caregivers, and other service providers. Communication with supervisors and interdisciplinary team members regarding patient progress, barriers, and treatment plans is essential.

Administrative responsibilities include completing comprehensive discharge summaries per regulatory requirements, documenting in the electronic system, maintaining accurate patient records, and recording treatment minutes according to all regulatory standards. You'll participate in patient care conferences, utilization review meetings, and rehabilitation meetings as required.

You may supervise and support physical therapist assistants, rehabilitation aides, and students. You must adhere to APTA standards, state practice acts, codes of conduct, and ethics requirements.
