Opportunity:

RN - Case Management

Job Details:

• Work Monday through Friday, 8:00 AM to 4:30 PM with potential weekend coverage as needed
• Floating policy based on scheduling needs across the facility
• Patient ratio of 1:21
• Perform daily Case Management assessments and evaluations
• Utilize computer skills with documentation systems including Meditech and CarePort
• 1-2 years of Case Management experience required
• Registered Nurse license required
• CCM certification preferred but not required
• Dress code: Burgundy scrubs or business casual
