PCU - Progressive Care Unit

RN - PCU

Job Details:

• Work 12-hour night shifts from 7:00 PM to 7:00 AM
• Schedule consists of 3 shifts per week (3x12s)
• Weekends and holidays required as needed
• Perform and document comprehensive patient assessments in accordance with RN scope of practice
• Consider patient culture, age, and developmental level when planning and delivering care
• Identify patient risk factors and presenting complaints
• Collect objective and subjective data and verify initial admission assessments
• Develop and prioritize patient problem lists in coordination with the inter-professional team
• Design and implement patient care interventions relevant to patient culture and age
• Determine desired patient outcomes in coordination with patients and families
• Explain procedures, treatments, and processes to patients and families
• Recognize, respond to, and communicate changes in patient condition appropriately
• Conduct and document ongoing assessments and responses to procedures and therapies
• Set priorities and coordinate plans of care for multiple patients
• Delegate tasks appropriately and oversee team members in patient care activities
• Monitor accuracy and completeness of delegated patient care provided by other team members
• Identify and address educational needs of patients based on level of comprehension
• Facilitate patient discharge process including discharge teaching and coordination with Case Management
• Actively participate in unit-based quality improvement activities including patient satisfaction, fall prevention, pressure ulcer prevention, and CAUTI monitoring
• Promote a positive group dynamic and healing environment through words and actions
• Apply evidence-based practice, current research, and caring philosophy to nursing practice
