Position Details

  • Schedule: 5 days per week, 8-hour days (no on-call or weekend requirements)
  • Weekly pay: $2,563.20
  • Facility type: Long-term care and rehabilitation center

Requirements

  • Massachusetts state physical therapy license
  • Minimum 1 year of physical therapy experience (new graduates with Level II clinical experience accepted)
  • Long-term care or rehabilitation experience required

Key Responsibilities

  • Screen, evaluate, and rehabilitate patients with physical functioning disorders and wound care needs
  • Develop and implement appropriate plans of care following regulatory and clinical practice standards
  • Utilize standardized assessments and evidence-based practice to support clinical interventions
  • Administer rehabilitation procedures including manual techniques, ambulation, therapeutic exercises, modalities, supportive and assistive devices, and wound care
  • Assess patient progress and adjust treatments accordingly in the clinical record
  • Evaluate treatment outcomes and modify services based on patient evaluation
  • Provide consultation and counseling to patients, families, caregivers, and service providers
  • Communicate effectively with supervisors and health team members regarding patient progress and treatment plans
  • Provide comprehensive discharge summaries per regulatory requirements
  • Document all required information in the electronic documentation system and maintain accurate patient records
  • Record treatment minutes for services provided according to regulatory standards
  • Participate in patient care conferences, utilization review meetings, and rehabilitation meetings
  • Supervise and support physical therapist assistants, rehabilitation aides, and students
  • Adhere to APTA and state required practice acts, code of conduct, and ethics
