Tide Health Group – Northwood Rehabilitation & Health Care Center is seeking a Physical Therapist for a long-term care position in Lowell, MA. This is a temporary assignment running 91 days, beginning July 27, 2026.

Position Details

  • Schedule: 5 days per week, 8-hour shifts (days)
  • No on-call or weekend requirements
  • Weekly pay: $2,741

Requirements

  • Massachusetts state physical therapy license required
  • Minimum 1 year of physical therapy experience
  • Long-term care or rehabilitation experience required
  • New graduates considered with Level II clinical experience

Key Responsibilities

  • Screen, evaluate, and rehabilitate patients with physical functioning disorders and/or wound care needs
  • Develop appropriate plans of care following all regulatory and clinical practice standards
  • Utilize standardized assessments and evidence-based practice to support clinical interventions
  • Administer various procedures including manual techniques, ambulation, therapeutic exercises, modalities, use of supportive and assistive devices, and wound care
  • Assess patient progress and adjust treatments accordingly in the clinical record
  • Evaluate treatment outcomes for effectiveness and modify services based on patient evaluations
  • Provide consultation and counseling to patients, families, caregivers, and other service providers
  • Communicate effectively with supervisors and health team members regarding patient progress, barriers, and treatment plans
  • Provide comprehensive discharge summaries per regulatory and clinical requirements
  • Document all required information in the electronic documentation system and maintain patient records per company policy
  • Accurately record treatment minutes for services provided per regulatory requirements
  • Participate in required meetings including patient care conferences, utilization review meetings, and rehabilitation meetings
  • Supervise and support physical therapist assistants, rehabilitation aides, and/or students
  • Adhere to APTA and state-required practice acts, code of conduct, and ethics
