Physical Therapist Job at Sylvan Grove Hospital

You will work as a Physical Therapist in the Rehabilitation Services department at Sylvan Grove Hospital. This is a day shift position running Monday through Friday, 8:00 AM to 4:30 PM, with one weekend day required every 4 weeks. The assignment is 91 days, beginning March 19, 2026, and pays $2,072 weekly.

Your responsibilities include:

• Assess patient conditions and develop individualized physical therapy treatment plans
• Implement rehabilitation programs to improve patient mobility and manage pain
• Select and execute appropriate treatment activities based on established plans
• Provide education and training to patients and their families
• Document evaluations, treatment sessions, and discharge plans according to departmental standards
• Collaborate with physicians to establish and refine treatment and discharge plans
• Maintain patient confidentiality and follow infection prevention protocols
• Attend staff meetings and participate in professional development and mandatory education
• Complete core and annual competencies on departmental timelines

Required credentials and skills:

• Bachelor's, Master's, or Doctorate degree in Physical Therapy
• Valid Physical Therapist license at time of hire
• BLS certification (American Red Cross Basic Life Support, or equivalent)
• Effective communication and customer service skills
• Basic computer proficiency
• Adherence to departmental dress code and ID badge requirements
