Tide Health Group – Southeast Rehabilitation & Skilled Care Center is seeking a Physical Therapist for a part-time, long-term care position in North Easton, MA. This 91-day assignment runs 5 days per week on the day shift with no on-call or weekend requirements.

About the Role

You will screen, evaluate, and rehabilitate patients with physical functioning disorders and wound care needs. Your responsibilities include:

  • Develop and implement appropriate plans of care aligned with regulatory and clinical practice standards
  • Utilize standardized assessments and evidence-based practice to guide clinical interventions
  • Administer manual techniques, therapeutic exercises, modalities, ambulation training, and wound care
  • Assess patient progress and adjust treatments accordingly with accurate clinical documentation
  • Evaluate treatment outcomes and modify services based on patient response
  • Provide consultation and counseling to patients, families, caregivers, and care team members
  • Communicate effectively with supervisors and interdisciplinary team members regarding patient progress and treatment plans
  • Generate comprehensive discharge summaries per regulatory requirements
  • Document all services in the electronic health record system and maintain accurate patient records
  • Record treatment minutes accurately per regulatory standards
  • Participate in patient care conferences, utilization review meetings, and rehabilitation planning sessions
  • Supervise and support physical therapist assistants, rehabilitation aides, and students

Qualifications

You must hold a valid Massachusetts state license and have a minimum of 1 year of physical therapy experience. Long-term care or rehabilitation experience is required. New graduates with Level II clinical experience are considered. You will adhere to APTA practice standards, state practice acts, and professional ethics throughout your assignment.

Compensation and Schedule

Weekly pay is $2,534.40. The assignment begins August 31, 2026.
